I'M GOING TO TELL YOU A LITTLE BIT ABOUT WHO WE ARE AND WHAT WE DO.

WE JUST OPENED A CENTER HERE IN HUBER HEIGHTS ON CHAMBERSBURG ROAD, BACK IN JANUARY.

SO WE'RE EXCITED ABOUT THAT.

SO THAT'LL BE A PART OF THIS PRESENTATION AS WELL, OBVIOUSLY, BUT WE PROVIDE MEDICAL PRIMARY CARE SERVICES, DENTAL, AND BEHAVIORAL HEALTH, AND WHAT WE REFER TO AS ENABLING SERVICES.

SO A PATIENT EDUCATION TRANSLATION ASSISTANCE WITH TRANSPORTATION AND FINANCIAL ELIGIBILITY FOR OUR PATIENTS AS WELL.

AND OUR MISSION STATEMENT HERE IMPROVING LIVES BY PROVIDING QUALITY, PRIMARY AND PREVENTIVE HEALTHCARE SERVICES TO THOSE IN NEED, REGARDLESS OF ABILITY TO PAY SIX CONVENIENT LOCATIONS.

UM, OBVIOUSLY THE MOST IMPORTANT NEAR TO YOU IS THERE ONE HERE IN CHAMBERSBURG ROAD, BUT WE HAVE ALEX CENTRAL AND MIAMI SUBURB.

AND THEN THE OTHER FOUR CENTERS ARE IN THE CITY OF DATE AND JUST A PICTURE OF OUR CENTERS THERE.

WE ALSO HAVE SOME SCHOOL-BASED SERVICES.

SO, UH, WE PARTNER WITH DAYTON, PUBLIC SCHOOLS ON A MOBILE HEALTH UNIT AND WE'RE IN THEIR SCHOOLS TWO DAYS A WEEK.

AND I'M HAPPY TO SAY WE GOT THAT RESURRECTED AFTER TWO YEARS OF COVID.

UM, BUT WE GO TO THE SCHOOLS TWO DAYS A WEEK PRO UH, PROVIDING CARE TO KIDS WITH ASTHMA IS OUR FOCUS THERE.

AND, UH, WE HAVE PARTNERSHIP WITH MIAMI VALLEY CHILD DEVELOPMENT CENTERS TO HEADSTART CENTERS.

SO WE'RE IN TWO OF THEIR CENTERS PROVIDING DENTAL SERVICES FOR THE, THE VERY YOUNG PATIENTS.

AND THERE'S THE MOBILE HEALTH UNIT AND OUR DENTAL TEAM THERE.

UH, SO REALLY EXCITED ABOUT THE CHAMBERSBURG HEALTH CENTER.

UM, YOU ALL PROBABLY KNOW BETTER THAN I DO.

I ALWAYS FORGET THE NAME OF THE DOCTOR WHO WAS THERE FOR SEVERAL, SEVERAL YEARS AND IT WAS SHUT DOWN FOR TWO YEARS.

SO WE TOTALLY GUTTED AND RENOVATED THAT FACILITY THERE.

IT TURNED OUT REALLY NICE.

SO I'M REALLY HAPPY WITH THE WAY IT WORKED OUT, NO 62 55 CHAMBERSBURG ROAD, RIGHT NEXT TO ST.

PETER'S CHURCH, IF YOU PICTURE OF 12 EXAM ROOMS AND, UH, TURNED OUT NICELY.

SO, UH, WE OPENED IN JANUARY WITH A PART-TIME NURSE PRACTITIONER, UH, NANCY HALL, UH, DR.

MONARCH WILL BE JOINING US NEXT MONTH.

AND I'M HAPPY ABOUT THAT.

DR.

MONARCH, I SEE PRACTICE 20 YEARS HERE IN, UH, HEBREW HEIGHTS AND CURRENTLY IS PRACTICING FOR THE LAST FIVE YEARS IN VANDELIA.

SO HER SCHEDULE IS ALREADY FULL FOR THE MONTH OF JUNE, JULY.

SO SHE'LL BE BRINGING A LOT OF PATIENTS WITH HER.

IT MAKES ME REALLY HAPPY.

UH, DR.

GATO IS ACTUALLY JOINING US FROM OUT OF TOWN.

SHE'S A PEDIATRICIAN, SO WE WILL HAVE TO DO A LOT MORE MARKETING, UH, TO FILL HER, HER SCHEDULE, BUT THE NEEDS THERE.

UM, I'M UNDERSTANDING.

SO, UH, OUR ORGANIZATION IS WHAT'S KNOWN AS A FEDERALLY QUALIFIED HEALTH CENTER, FQHC, YOU KNOW, UH, REALLY CREATED THIS PRESENTATION A LONG TIME AGO TO EDUCATE FOLKS ON JUST WHAT IT MEANS TO BE AN FQHC.

MOST FOLKS HAVE NO CLUE WHAT THAT MEANS.

WHAT ASPHALT GO INTO A LITTLE BIT HERE.

THERE WAS ABOUT 1400 ORGANIZATIONS ELECT, UH, LIKE US AROUND THE COUNTRY, 55 HERE IN OHIO.

UM, WE ALL GET SOME FEDERAL FUNDING.

UH, IT'S ABOUT 15% OF OUR BUDGET.

IT HELPS, BUT A SMALL PERCENTAGE OF OUR BUDGET.

THE BIG PRIZE FOR US IS THE ENHANCED REIMBURSEMENT.

I REFER TO IT AS APPROPRIATE REIMBURSEMENT.

SO THAT IS FOR PATIENTS THAT WE SEE WHO ARE ON MEDICAID AND MEDICARE, MAINLY THE MEDICAID PATIENTS.

SO WITH THAT GRANT AND THAT ENHANCED REIMBURSEMENT, WE HAVE REQUIREMENTS.

WE HAVE TO SEE PATIENTS REGARDLESS OF THEIR ABILITY TO PAY.

SO IF WE'RE A, IF PATIENTS ARE UNINSURED, THAT'S OKAY, WE GLADLY ACCEPT MEDICAID.

WE GLADLY ACCEPT ALL THE COMMERCIAL PLANS AS WELL.

ANTHEM, ADMIN, HUMANA, YOU NAME IT, WE TAKE IT, BUT WE CAN'T TURN ANYBODY AWAY BASED ON INABILITY TO PAY.

WE, WE MENTIONED WE PROVIDE PRIMARY CARE, DENTAL, BEHAVIORAL HEALTH, AND ENABLING SERVICES.

WE OFFER A SLIDING FEE SCALE DISCOUNT BASED ON FAMILY SIZE AND INCOME AS WELL.

AND I, YOU KNOW, UM, SO AS, UH, WE ALSO ARE, UH, OUR ORGANIZATION IS A NONPROFIT AND, YOU KNOW, I ALWAYS CHUCKLE WHEN I HEAR SOME, UH, FOLKS IN THE NONPROFIT WORLD SAY, YEAH, WE HAVE A CONSUMER ON OUR BOARD OF DIRECTORS WHILE I AM REQUIRED TO HAVE A PATIENT MAJORITY BOARD OF DIRECTORS.

SO I HAVE 10 BOSSES AND SIX OF THEM ARE PATIENTS OF ONE OF OUR CENTERS.

SO IT KEEPS ME HONEST AND MAKES MY JOB VERY INTERESTING.

WE HAVE THE OPTION OF PROVIDING A PHARMACY WITHIN OUR FOUR WALLS OR PARTNERING WITH, UH, OTHER PHARMACIES CONTRACT PHARMACIES.

AND WE PARTNER WITH, WITH ALL OF THOSE PHARMACIES YOU SEE THERE.

SO OUR PATIENTS GET A DISCOUNT, UM, ABOUT A 20 TO 50% DISCOUNT OFF THE NAME BRAND DRUGS.

IF THEY GO TO ONE OF THOSE PARTICIPATING PHARMACIES, UH, LAST YEAR WE SAW OVER 21,000 PATIENTS, INDIVIDUALS, AND THEY MADE ABOUT 54,000 VISITS.

SO THAT WAS A LITTLE BIT OF A DIP FROM, UH, 2020, UM, ACTUALLY FROM 2019, UH, JUST BECAUSE OF COVID STILL HAD AN EFFECT, BUT, UH, IT'S OUR PAYER MIX THERE.

THE BULK OF OUR PATIENTS ARE ON MEDICAID.

MEDICARE.

THE ELDERLY IS 21%.

UM, 13% HAVE SOME KIND OF PRIVATE INSURANCE OR COMMERCIAL INSURANCE AND ONLY ABOUT 6% ARE UNINSURED.

SO WE'RE HAPPY ABOUT THAT.

THAT JUST SHOWS THE TRENDS OVER THE YEARS.

YOU SEE THE DIP THERE IN 2020, AND THEN IT SPIKED A LITTLE BIT LAST YEAR, MAINLY BECAUSE OF BACKSEAT, WHERE WE GAVE OUT A LOT OF COVID VACCINES LAST YEAR.

SO A LOT OF THOSE PATIENTS WERE ONE TIME AND OUT.

THEY PROBABLY WON'T SEE THEM AGAIN.

SO THAT'S CORRECT, VERY PROUD OF THE QUALITY WE PROVIDE.

SO IN 2020, WE WILL RECEIVE A BRONZE AWARD, WHICH SAYS WE WERE IN A 30% OF ALL HEALTH CENTERS ACROSS THE COUNTRY IN TERMS OF OUR CLINICAL QUALITY OUTCOMES.

UH, IT GOT EVEN BETTER LAST YEAR, WE ARE IN

THE TOP 20%.

SO WE GOT THE SILVER AWARD, UH, FOR OUR CLINICAL OUTCOMES OF ALL THE HEALTH CENTERS ACROSS THE COUNTRY.

SO I'M REALLY HAPPY ABOUT THE FACT THAT WE PROVIDE A GOOD QUALITY CARE.

IN ADDITION, UH, WE WERE ASKED BY THE AMERICAN CANCER SOCIETY, IF WE WOULD APPLY FOR A GRANT, UH, AS THE BREAST HEALTH EQUITY GRANT, AND THEY AWARDED IT TO NINE GRANTEES AROUND THE COUNTRY, AND WE WERE HAPPY TO BE A ONE OF THOSE VERY PRESTIGIOUS ORGANIZATIONS LISTED THERE, UM, THAT MOST FOLKS HAVE HEARD OF.

AND THEN THERE'S US WHO PROBABLY NOBODY'S HEARD OF OUTSIDE OF THE DAYTON AREA.

SO, UM, BUT IT WAS A, A TESTAMENT TO THE QUALITY WE PROVIDE.

AND THIS GRANT IS SPECIFICALLY GEARED TOWARDS, UH, LEVELING THE PLAYING FIELD, UH, FOR, UH, BREAST HEALTH AND BREAST CANCER MORTALITY.

SO AFRICAN-AMERICAN WOMEN DIED A 40% HIGHER RATE FROM BREAST CANCER THAN OTHER WOMEN, EVEN THOUGH YOU GET YOUR MAMMOGRAMS AT A SIMILAR RATE.

SO, UH, WE RECEIVED THIS GRANT IN JANUARY OF 2020, UH, WITH THE GOAL OF, UH, FIGURING OUT HOW TO BRIDGE THAT GAP AND THEN COVID HIT, BUT, UH, WE'RE STILL WORKING VERY HARD TO, TO WORK, UH, EVEN THE PLAYING FIELD, SO TO SPEAK, UH, THERE'S THE GOALS.

AND I REALIZED THERE WAS A TYPO IN THERE THAT SHOULD HAVE SAID WE WENT INCREASED BREAST CANCER SCREENING RATES FOR ALL WOMEN BY 10% AND LEE INSTEAD OF FIVE AND MORE SPECIFICALLY INCREASING OUR SCREENING RATES FOR AFRICAN-AMERICAN PATIENTS BY 10% AS WELL.

UH, WE WANTED TO BUILD A COALITION, A COMMUNITY PRESENCE, UH, FOR, UH, EFFECTIVE MARKETING AND EDUCATION AROUND OUR TARGET AUDIENCE, AND THEN, UH, CREATE AN ADVISORY TEAM THAT WORKS CLOSELY TOGETHER.

SO WE'VE DONE A REALLY GOOD JOB.

WE'VE CREATED THAT ADVISORY TEAM, GOT THE HOSPITAL SYSTEMS. WE'VE GOT THE NORMAL JAGER OFF MEMORIAL FOUNDATION, AND WE'VE GOT SEVERAL OTHER PARTNERS, PINK RIBBON, UH, LADIES ON THERE.

UH, SO A GOOD TEAM OF FOLKS WORKING ON THIS.

UH, WE, UH, HAVE PLANNED A LOT OF IN-PERSON EVENTS THAT JUST COULD NOT HAPPEN.

SO WE REALLY SHIFTED TO A LOT OF MARKETING, A LOT OF DIGITAL MARKETING, UH, FACEBOOK BILLBOARDS, UH, SNAIL MAIL, YOU NAME IT.

SO MARKETING HAS REALLY HAD, HAD BEEN OUR FOCUS BECAUSE WE COULDN'T GET TOGETHER IN PERSON LIKE WE WANTED TO, BUT WE ARE PLANNING AN EVENT THIS YEAR TO GET TOGETHER AND HAVE A CONFERENCE AROUND, UH, FIGHTING AGAINST BREAST CANCER.

WE RENAMED IT BAB.

SO THAT'S OUR INITIATIVE THERE.

UM, I DON'T KNOW IF THIS WILL THAT, CAN I, CAN YOU CLICK ON IT, HAVE THAT PLATE.

SO WE HAD A MET THE CHAMBERSBURG HEALTH CENTER.

THERE'LL BE A CHARLES DREW AND NEXT WEEK, AND LATER THIS MONTH AT ANOTHER ONE OF OUR CENTER.

SO IF, UH, WE KNOW OUR PATIENTS WHO ARE OVERDUE FOR THEIR MAMMOGRAMS, SO WE'RE FLOODING THEM WITH SNAIL MAIL AND TEXT MESSAGES TO TRY TO GET THEM OUT AND GET THE MESSAGE OUT, GET THE WORD OUT.

BUT, UH, IT'S, IT'S A STRUGGLE.

SO, UH, WE ALSO HAVE A CAMPAIGN FOR THE MEN.

SO THIS WAS LAST YEAR'S CAMPAIGN.

IT'S A PUBLIC SERVICE CAMPAIGN THAT FEATURED RADIO TV AND PRINT ADS.

UM, GUYS, WE KNOW THAT, UM, OUR DATA SHOWS US AND IT'S THE SAME FOR EVERY YEAR.

SO THIS IS ALL THOSE ORGANIZATIONS LIKE US AROUND THE STATE.

THIS IS STATEWIDE DATA FOR ABOUT 15 HEALTH CENTERS FROM 2019.

AND SO THIS WAS PRE COVID AND IT SHOWS THE AGE OF THE VISITS.

SO IF YOU LOOK AT THOSE NUMBERS THERE, IT DOESN'T MATTER THE AGE UNTIL YOU GET TO ABOUT 16 OR 17.

THEY'RE PRETTY EVEN NUMBER OF VISITS, UH, FOR THE GUYS AND THE GIRLS OF LATE THE YOUNG LADIES AND YOUNG MEN, AND ABOUT AGE 19 WOMEN VISITS ARE TWICE THE MALE VISITS AND IT STAYS THAT WAY FOR SEVERAL YEARS.

SO MOST LADIES KNOW WHAT I'M TALKING ABOUT.

WHEN I SAY THIS, THEY KNOW EXACTLY THEY GO, YEP.

MY HUSBAND.

MY SON.

YEAH.

HALF EASY SLAVES.

THEY CUT HER HEAD.

AND DURING THOSE YEARS I KNEW MY NUMBERS.

NOW WE DON'T HAVE MORTALITY RATES FOR OUR CENTER IN OUR PATIENTS DON'T CALL AND SAY HE GOT DIED.

WE DON'T KNOW THAT, BUT NATIONAL DATA TELLS US, UH, THAT MORTALITY RATES HIGHER.

UM, I THINK IT IS GETTING A MAMMOGRAM AT A LATER STAGE IS A FACTOR.

AND THEN NOT FOLLOWING UP, IF YOU HAVE AN ABNORMAL MAMMOGRAM FEAR, PART OF THAT IS FEAR OF THE DISTRUST IN THE MEDICAL SYSTEM, WHICH GOES BACK MANY, MANY YEARS, RIGHT.

HISTORICALLY WITH TUSKEGEE AND ALL THOSE GOOD THINGS.

SO, UM, YOU KNOW, VACCINE HESITANCY WITH COVID SHOWED THAT AGAIN, WHERE PEOPLE OF COLOR WERE MORE HESITANT TO GET A VACCINE.

SO IT'S, SOMEONE'S FEAR.

YOU KNOW, WE TAKE TRANSPORTATION OUT OF THE EQUATION CAUSE WE BRING THEM THE MAMMOGRAPHY TO OUR CENTERS TO THE SAME PLACE.

SO OVERALL, THE PURPOSE OF THIS FORUM IS TO DISCUSS THE PROCLAMATION THAT CITY COUNCIL HAD VOTED ON BACK ON LAST APRIL, ACTUALLY APRIL 1ST, 2021.

AND THERE WERE SEVEN SECTIONS IN THAT PROCLAMATION THAT FOCUS ON VARIOUS THINGS, PRETENDING TO DNI INITIATIVES, SUCH AS EQUITY AND FAIR HIRING PRACTICES AMONG MINORITY CANDIDATES FOR VARIOUS CITY STAFF POSITIONS AND POLICIES THAT NEED TO BE ENFORCED.

WE ARE STRONGLY ENCOURAGING THESE THREE PANEL MEMBERS, THE COMMISSION ANSI COUNCIL TO HAVE A PRODUCTIVE AND ENGAGING CONVERSATION ON COURSES OF ACTION THAT NEED TO BE PUT IN PLACE.

AND A GOOD EXAMPLE FOR WHAT WE SAW FROM THE CURRENT HIRING OF OUR NEW CITY MANAGER.

SO THAT IS DEFINITELY A NEED TO ENFORCE THIS TYPE OF DISCUSSION.
